2021/22 CCG Practice agreements

The IT/Digital team have issued the 2021/22 CCG Practice agreements which detail the provision of IT and Digital services commissioned by the CCG on behalf of your practice (via email). The document sets out responsibilities as aligned in the Securing Excellence in Primary Care – Digital Services Operating Model – for practices, PCNs and CCGs.

Upon review of the agreement, please can the agreement signature page be countersigned by the Practice’s authorised signatory and returned electronically to us at [email protected] by no later than, Friday 16th July 2021.
